Addictive responses to various substances and behaviors like sex, eating, and gambling share many behavioral commonalities. These commonalities, such as craving, compulsivity, and withdrawal syndromes can be exploited by the clinician who delivers treatments directed at the specific behavior pattern.
Although it is a commonplace that addiction and mental illness run together, clinicians and scientists must ask themselves the following; why do the two conditions co-occur so regularly? Karen Hartwell, MD, and colleagues present an outline of the interactions between mental illness and addiction related to stress and the hypothalamic-pituitary-adrenal axis, glutamate, dopamine, g-aminobutyric acid, norepinephrine, and serotonin. They summarize and interpret the available studies and make recommendations for future research and public policy change, but their article is useful in the here and now for practitioners who struggle both to understand the biology of these vexing conditions and to explain it to patients and their families.
In addition to the biologic underpinnings of their conditions, patients often wonder about their genetics both in relation to their own loading and for a quantification of risk to their own children. Edwards, PhD, and colleagues address this very issue in their article on the genetics of addiction. In this complete yet easily understandable article, they outline the current understanding of the heritability of addictive behaviors.
By reviewing the available twins studies and explaining the techniques of gene linkage analysis and association studies of candidate genes and genome-wide assays, the authors describe the present state of knowledge about the genetic influence on addiction.
